Once you have reviewed our referral process AND discussed the case with the Duty Worker, please complete this form and email it to
dutyworker@scotscare.com. A hard copy of the form should then be enclosed with the clients fully completed General Application form

Please ensure that your client fully completes this Application Form and send it with a copy of the Agency Referral Form that you emailed to the ScotsCare Duty Worker. It will save time if you can enclose proof of your clients benefits/income and Scottish connection.
